  • Tell a Story:  Structure your case study like a story, with a clear beginning (the problem), middle (the solution), and end (the results). A narrative approach keeps the audience engaged. 
  • Focus on the Problem and Solution:  Clearly articulate the problem you addressed and how your solution was unique or effective. This section is the core of a case study and should be given ample attention. 
  • Use Data Wisely:  Incorporate relevant data to support your points, but avoid overwhelming the audience with numbers. Use charts and graphs for visual representation of data to make it more digestible. 
  • Highlight Key Results:  Emphasize the impact of your solution with clear and quantifiable results. This could include increased revenue, cost savings, improved customer satisfaction, and similar benefits.
  • Incorporate Visuals:  Use high-quality visuals to break up text and explain complex concepts. Consider using photos, infographics, diagrams, or short videos.  “I put together the graphics that tell the story visually. Speakers often just need a big image or charts and graphs to help guide their talk track.   • Include Testimonials:  Adding quotes or testimonials from clients or stakeholders adds credibility and a real-world perspective to your presentation. 
  • Practice Storytelling:  A well-delivered presentation is as important as its content. Practice your delivery to ensure you are clear, concise, and engaging. At this point, it also makes sense to solicit feedback from stakeholders. Jaquet concurs: “Once my outline and graphics are in place, I typically circulate the presentation draft for review. The feedback step usually surfaces nuances in the story or key points that need to show up on the slides. There is no point in building out tons of slides without alignment from the speaker or subject matter experts.”
  • End with a Strong Conclusion:  Summarize the key takeaways and leave your audience with a final thought or call to action. 
  • Seek Feedback:  After your presentation, request feedback to understand what worked well and what could be improved for future presentations.

It’s one of the few places where you can properly begin to talk about all the great stuff you do in your work, without coming across like an egotistical nightmare. Case studies in your presentation can be used to get your audience excited about working with you. And to demonstrate that you can be trusted – that you’ve been there, done that, and got the undeniable results.

Ask how the process was for them. And, of course, talk about their new world and how much better it is now.  

It’s nothing personal, but your audience is much more likely to believe the results, if someone else is telling them. 

But once you have that information from your client, how do you turn it into a story? We know, we know – we harp on about storytelling all the damned time. But the importance of utilising narrative structure here can’t be understated. And it’s really easy to do. 

What’s the best case study structure? 

There are no hard and fast rules when it comes to the structure of your case study presentation. Every client journey is unique, so – when you present your case study – you should reflect that. Having said that, there are some key ingredients that most case studies need to include to have the greatest impact. And following a set structure just makes the whole thing a lot easier, eh? 

  • Set the scene 

Pique your audience’s interest with an emotional hook, setting the scene and getting them invested in the client story. 

Who is the client you’re talking about? What industry do they sit inside? Why should your audience care? Is there any relevant history that your audience could benefit from hearing?  

  • What challenges darkened their door? 

Why did they need to hire you? What difficulties were they facing before you came along – either personally or across their industry? Did they have a specific goal they wanted to achieve, with your help? 

  • How did your solution save the day? 

What were you able to bring to the table that nobody else could? What unique approach did you use to find your angle of attack?  

One ballsy approach could be to outline any parts that went pear shaped, and how you fixed them. We learn the most valuable lessons rectifying the chaos of a mistake. There’s a certain charm in owning up to your mistakes that will humanise you to your audience. And that never hurts when you’re trying to get them to buy into your service. Just make sure you don’t forget to tell them what you learned, and what’s in place now to ensure it will never happen again. 

  • Rustle up some results 

Once you’ve grabbed their attention, drive the weight of the case study home with transformational facts and figures. Demonstrate the real and measurable change your client was able to effect with you by their side.  

Are there any tangible stats you can fling into the faces of your audience members? An increase in traffic? An uptick in sales? A boost in brand visibility?  

If you don’t have any substantial stats, you could frame a picture of the future using the results your service has provided as a springboard. Just make sure to keep them grounded in reality.

When’s the best time to present my case study? 

Again, every audience – and every presentation – is different, but – 90% of the time – the best place for a case study is towards the end of your presentation. By this point, you’ll have pulled on their emotional responses, shown you have a solution that their gut says is perfect for them, but they’ll be looking for something credible to seal the deal. 

Try to move seamlessly from presenting the solution to backing it up with a relevant case study. And you may want to think about including one case study per solution, so that you’re never lacking for proof points.  

How to present a case study slide. 

You’ve written your case study story. You know where it’s going to go in your deck. Now you just need to build the thing.  

Remember that all your presentation principles should still hold true for this portion of your deck. Stick to one big idea per page, minimal on-slide copy, engaging visuals, and rich detail hiding inside the speaker notes. 

Your case study may warrant one slide per stage of the structure. Or all four stages might be more impactful on one. It all depends on how many ideas you’re trying to communicate, and how overwhelming it all could be if smushed together.

  • The interviewer will first ask how you intend to analyze and evaluate the problem’s underlying issues.

A third element of the analysis could be to understand the effect of education in Country X’s economics objectives.

Step 1: Formulate the right question

Glanzmann thrombasthenia (GT) is a rare platelet disorder characterized by qualitative/quantitative deficiencies of the platelets’ fibrinogen receptor, glycoprotein (GP) IIb/IIIa complex, resulting in impaired platelet aggregation and increased bleeding time. Most cases are hereditary with an autosomal recessive pattern of inheritance, but acquired GT also occurs. We report the surgical management of symptomatic chronic subdural hematoma (CSDH), a rare condition in young individuals, in a 37-year-old man who had GT and a history of mild head trauma approximately one month before admission. Despite hematologic consultation, normal bleeding time and clotting time, and platelet transfusion before surgery, massive hemorrhage during surgery, epidural hematoma, and anisocoria in the ICU occurred that led to craniectomy. This report highlights that CSDH management in patients with GT requires close monitoring of these patients as well as collaboration between neurosurgeons, intensive care physicians, hematologists, and anesthesiologists.

Glanzmann thrombasthenia (GT) is a rare bleeding disorder featured with qualitative or quantitative deficiencies of a fibrinogen receptor (glycoprotein (GP) IIb/IIIa) on the platelets, leading to absence of/impaired platelet aggregation, decreased clot retraction, and increased bleeding time while the platelet count is low to normal [ 1 ]. Although GT is mostly an inherited autosomal recessive disorder, acquired GT may also occur due to auto-immunization [ 1 ]. GT has a prevalence of one per million individuals and is more frequent in countries with a higher incidence of consanguinity [ 2 ].

Chronic subdural hematomas (CSDH) are liquefied hematomas in the subdural space with a characteristic outer membrane that occurs a few weeks after head injury and one of its risk factors is thrombopathy [ 3 ]. CSDH are typically seen in the elderly population and are rare in young adults [ 4 ]. The most common theory for the cause of CSDH is that minor inertial brain injury causes movement of the brain within the skull and tears bridging veins as they traverse the cell layer of the dural border [ 3 ]. Without timely interventions, CSDH may be fatal despite their benign nature [ 4 ]. It is consensus that patients with symptoms that can be attributed to radiologically confirmed CSDH should be treated [ 3 ].

Platelet transfusion is the standard treatment for patients with GT as a countermeasure for major surgeries. Recombinant activated factor VII (rFVIIa) has also been used in patients with antiplatelet antibodies that render transfusion ineffective [ 5 ]. A lack of scientific knowledge of rare diseases and their appropriate management may result in poor treatment outcomes. Thus, this study aimed to report the case of a young male with GT who had symptomatic CSDH.

A 37-year-old man was admitted to the emergency department of Taleghani Hospital (Kermanshah, Iran) with complaints of intractable new onset headache, nausea, vomiting, and confusion. Approximately 4 weeks before admission, he had a mild head trauma. Past medical history was remarkable for opioid abuse, intermittent spontaneous otorrhagia controlled by repeated platelet transfusion and GT. However, his laboratory data was unremarkable.

On the examination at admission, he showed normal vital signs. Although the patient’s score on the Glasgow Coma Scale (GCS) was 13/15, no other remarkable abnormalities were found by neurological examination. Due to impaired consciousness and a history of head injury, the patient underwent a brain CT scan that revealed a significant hypo-dense left frontoparietal hematoma and the CSDH were diagnosed. Thus, a burr-hole craniostomy was performed.

Although the preoperative testings showed normal bleeding time (BT) and clotting time (CT), the patient received 5 units of platelet preoperatively after consultation with a hematologist. Surgery was started with two burr holes in the frontoparietal area. The hematoma was evacuated but massive and uncontrollable hemorrhage was seen through the bone and scalp. As possible, partial hemostasis was achieved by bone wax and surgical hemostasis dressing (surgicel) and bipolar cautery, drains were fixed, and the scalp was stitched.

Then, the patient was admitted to the intensive care unit (ICU) by concomitant packed cell transfusion. However, his GCS score began to decrease and he became anisocoric. Immediately, red blood cell and platelet transfusion was done and reoperation was conducted by craniectomy. The epidural and subdural hematoma was evacuated and hemostasis was acceptably achieved by surgicel, bone wax, and bipolar. The dural tack-up stitch was done, the drain was fixed and the scalp was stitched by continuous vertical mattress technique ( Fig. 1 ). Due to the possibility of massive hemorrhage during subcutaneous bone flap storage, the bone flap was discarded. During the craniectomy and until 48 h, the patient was infused with intravenous tranexamic acid 2 gr every 6 h and fibrinogen 1 gr every 8 h. He spent 5 days in the ICU and 5 days on the ward before discharge. From the first surgery until discharge, he received 35 units of pooled platelet, 7 packed cells, and 4 FFP.

One month later, he was admitted because of a headache and the CT scan showed epidural hematomas ( Fig. 2 ). Hence, he was taken to the operating room for hematoma evacuation. In the laboratory examination, increased BT (10 min) was found, and 2 units P.C., 2 units single donor platelet (SDP), and 33 units random donor platelet (RDP) were transfused preoperatively, leading to decreased BT (4 min). After surgery, he received 2 units P.C., 2 units platelet, and 5 units RDP and was discharged home after 5 days.

Two months later, he was admitted for cranioplasty, and a hematology and anesthesiology consult was done. BT and CT were normal but perioperatively he received 35 units RDP, 2 units SDPs, and 5 units P.C. Despite a huge platelet transfusion before and after cranioplasty, bleeding in the ward was reported to the neurosurgery resident by the nurse. It wasn’t controlled by overrunning stitches because of bleeding in the needle insertion sites. Finally, bleeding was controlled by tight elastic bandages. After 5 days, the patient was discharged. On follow-up at 2 months, he had no neurologic deficit or residual symptoms.

GT is an extremely rare inherited disorder of platelet function with an autosomal recessive mode of inheritance. However, acquired GT due to autoimmunization has also been reported. GT is characterized by qualitative/quantitative deficits of a platelet’s fibrinogen receptor, GP IIb/IIIa, resulting in impaired platelet aggregation and increased bleeding time [ 1 ]. This rare disorder is more common in populations with a higher rate of consanguinity [ 2 ]. Common clinical manifestations of GT include spontaneous mucosal and skin bleeding [ 6 ]. There are three subtypes for GT, but no correlation has been found between the bleeding severity and the GT subtype [ 7 ].

No therapy is required in patients with GT regularly. However, treatment is always required for these patients during spontaneous bleeding episodes as well as surgical procedures, which are significant challenges due to bleeding probability and a high incidence of alloimmunization induced by repeated platelet transfusion [ 1 , 7 ]. Local measures (e.g. local pressure, ice therapy, topical thrombin, fibrin sealants, etc.) and/or antifibrinolytic agents (e.g. tranexamic acid) are usually effective in the case of mild to moderate bleeding episodes. Platelet transfusion is the standard treatment for moderate to severe bleeding and the routine prophylaxis for major surgeries in patients with GT [ 1 ]. Alternative treatment with rFVIIa has also been recommended for patients who are alloimmunized, but it is expensive and associated with the risk of thrombosis [ 1 , 7 , 8 ].

CSDH are usually caused by slow bleeding from bridging veins on the cerebral surface. However, its pathophysiology has not yet well been understood. While CSDH is one of the most common cases of neurosurgery, it is usually seen in elderly subjects and is rare in young individuals [ 3 ]. Its management is based on the presence and severity of hematoma-related symptoms and hematoma size and mass effect. Surgery is required for symptomatic CSDH, hematoma thickness > 10 mm, or midline shift > 7 mm [ 9 ]. The burr-hole craniostomy with/without a drainage system is the most common and most efficient surgical technique used for CSDH management. In the case of hematoma recurrence, burr-hole conversion into a craniotomy should be considered [ 9 ]. CSDH is a benign condition, nonetheless, it may be fatal without prompt surgical intervention and hematoma recurrence is still an important concern [ 4 ].

To the best of the authors’ knowledge, there are no case reports of CSDH management in young patients with GT. As mentioned previously, the classical measures were done in our case, but simple neurosurgery for hematoma evacuation became complicated and nearly fatal despite the teamwork of the neurosurgeon, hematologist, and anesthesiologist. Unfortunately, there are no established recommendations for treating and prophylaxis of surgical bleeding in patients with GT and a combination of functional platelet tests and clinical assessment is used for assessing the response to treatment [ 10 ]. However, a useful management algorithm has been proposed by Solh et al. for elective and urgent surgical interventions in these patients [ 1 ].

We reported a rare case of a young patient with a CSDH who also had rare GT and a history of repeated platelet transfusion to control spontaneous otorrhagia. This report highlights the need for close monitoring in patients with GT when neurosurgery is done for CSDH management. One important point is that despite having normal BT and CT, the patient’s bleeding during and after the surgery was difficult to control. Therefore, as previous studies have also shown, BT and CT are poor predictors of clinical bleeding. In light of our experience with this case, we suggest if a patient with CSDH and GT is neurologically and hemodynamically stable, the neurosurgeons should be more prudent and perform more investigations as needed, like what was suggested by Solh et al. Further, close collaboration between neurosurgeons, intensive care physicians, hematologists, and anesthesiologists is critical for CSDH management in patients with GT.

Black History Month 2024: African Americans and the Arts

The national theme for Black History Month 2024 is “ African Americans and the Arts .”  

Black History Month 2024 is a time to recognize and highlight the achievements of Black artists and creators, and the role they played in U.S. history and in shaping our country today.  

To commemorate this year’s theme, we’ve gathered powerful quotes about learning, culture and equality from five historic Black American authors, teachers and artists who made a significant impact in the Arts, education ― and the nation.  

  Making history  

“Real education means to inspire people to live more abundantly, to learn to begin with life as they find it and make it better.” – Carter G. Woodson, Author, Journalist, Historian and Educator, 1875-1950  

Known as the “Father of Black History,” Carter G. Woodson was primarily self-taught in most subjects. In 1912, he became the second Black person to receive a Ph.D. from Harvard.   

He is the author of more than 30 books, including “T he Mis-Education of the Negro. ”  

Carter G. Woodson dedicated his life to teaching Black History and incorporating the subject of Black History in schools. He co-founded what is now the Association for the Study of African American Life and History, Inc. (ASALH) . In February 1926, Woodson launched the first Negro History Week , which has since been expanded into Black History Month.

Providing a platform  

“I have created nothing really beautiful, really lasting, but if I can inspire one of these youngsters to develop the talent.” – Augusta Savage, Sculptor, 1892-1962  

An acclaimed and influential sculptor of the Harlem Renaissance, Augusta Savage was a teacher and an activist who fought for African American rights in the Arts. She was one out of only four women, and the only Black woman, commissioned for the 1939 New York World’s Fair. She exhibited one of her most famous works, “Lift Every Voice and Sing,” which she named after the hymn by James Weldon Johnson, sometimes referred to as the Black National Anthem. Her sculpture is also known as “ The Harp, ” renamed by the fair’s organizers.

Raising a voice  

“My mother said to me ‘My child listen, whatever you do in this world no matter how good it is you will never be able to please everybody. But what one should strive for is to do the very best humanly possible.’” – Marian Anderson, American Contralto, 1897-1993  

Marian Anderson broke barriers in the opera world. In 1939, she performed at the Lincoln Memorial in front of a crowd of 75,000 after the Daughters of the American Revolution (DAR) denied her access to the DAR Constitution Hall because of her race. And in 1955, Marian Anderson became the first African American to perform at the Metropolitan Opera. She sang the leading role as Ulrica in Verdi’s Un Ballo in Maschera.

Influencing the world  

“The artist’s role is to challenge convention, to push boundaries, and to open new doors of perception.” – Henry Ossawa Tanner, Painter, 1859-1937  

Henry Ossawa Tanner is known to be the first Black artist to gain world-wide fame and acclaim. In 1877, he enrolled at the Pennsylvania Academy of the Fine Arts , where he was the only Black student. In 1891, Tanner moved to Paris to escape the racism he was confronted with in America. Here, he painted two of his most recognized works, “ The Banjo Lesson” and “ The Thankful Poor of 1894. ”    

In 1923, Henry O. Tanner was awarded the Chevalier of the Legion of Honor by the French government, France’s highest honor.

Rising up  

“Wisdom is higher than a fool can reach.” – Phillis Wheatley, Poet, 1753-1784  

At about seven years old, Phillis Wheatley was kidnapped from her home in West Africa and sold into slavery in Boston. She started writing poetry around the age of 12 and published her first poem, “ Messrs. Hussey and Coffin ,” in Rhode Island’s Newport Mercury newspaper in 1767.   

While her poetry spread in popularity ― so did the skepticism. Some did not believe an enslaved woman could have authored the poems. She defended her work to a panel of town leaders and became the first African American woman to publish a book of poetry. The panel’s attestation was included in the preface of her book.  

Phillis Wheatley corresponded with many artists, writers and activists, including a well-known 1 774 letter to Reverand Samson Occom about freedom and equality.

Purpose: New-Onset Refractory Status Epilepticus (NORSE) is a rare and severe form of refractory status epilepticus without an apparent underlying cause at presentation or prior history of epilepsy. We aimed to describe the clinical features, etiology, treatment, and outcomes of NORSE in adults in a quaternary-level hospital in Saudi Arabia.

Methodology: In this retrospective cohort study, inclusion criteria involved patients over 14 years old who met the 2018 consensus definition for NORSE. Patients were identified using a combination of medical record admission labels 'status epilepticus' and 'encephalitis', and continuous EEG reports documenting status epilepticus. Demographic, clinical, and radiological data were collected and then analyzed for factors correlated with specific etiologies, better functional outcomes, and future diagnosis of epilepsy.

Results: We found 24 patients presenting with NORSE between 2010 and 2021. Fever/infectious symptoms were the most common prodrome. Elevated inflammatory serum and cerebrospinal fluid markers in most patients. Brain MRI revealed T2/FLAIR hyperintensity patterns, predominantly affecting limbic and perisylvian structures. The etiology of NORSE varied, with immune-related causes being the most common. Long-term outcomes were poor, with a high mortality rate and most survivors developing drug-resistant epilepsy.

Conclusion: This study provides valuable insights into NORSE's clinical characteristics, highlighting the heterogeneity of this condition. The poor outcome is likely related to the progressive nature of the underlying disease, where refractory seizures are a clinical symptom. Thus, we propose to focus future research on the etiology rather than the NORSE acronym.

The agency is loosening its covid isolation recommendations for the first time since 2021 to align it with guidance on how to avoid transmitting flu and RSV, according to four agency officials and an expert familiar with the discussions.

CDC officials acknowledged in internal discussions and in a briefing last week with state health officials how much the covid-19 landscape has changed since the virus emerged four years ago, killing nearly 1.2 million people in the United States and shuttering businesses and schools. The new reality — with most people having developed a level of immunity to the virus because of prior infection or vaccination — warrants a shift to a more practical approach, experts and health officials say.

“Public health has to be realistic,” said Michael T. Osterholm, an infectious-disease expert at the University of Minnesota. “In making recommendations to the public today, we have to try to get the most out of what people are willing to do. … You can be absolutely right in the science and yet accomplish nothing because no one will listen to you.”

The CDC plans to recommend that people who test positive for the coronavirus use clinical symptoms to determine when to end isolation. Under the new approach, people would no longer need to stay home if they have been fever-free for at least 24 hours without the aid of medication and their symptoms are mild and improving, according to three agency officials who spoke on the condition of anonymity to share internal discussions.

The federal recommendations follow similar moves by Oregon and California . The White House has yet to sign off on the guidance that the agency is expected to release in April for public feedback, officials said. One agency official said the timing could “move around a bit” until the guidance is finalized.

Work on revising isolation guidance has been underway since last August but was paused in the fall as covid cases rose. CDC director Mandy Cohen sent staff a memo in January that listed “Pan-resp guidance-April” as a bullet point for the agency’s 2024 priorities.

Officials said they recognized the need to give the public more practical guidelines for covid-19, acknowledging that few people are following isolation guidance that hasn’t been updated since December 2021. Back then, health officials cut the recommended isolation period for people with asymptomatic coronavirus from 10 days to five because they worried essential services would be hobbled as the highly transmissible omicron variant sent infections surging. The decision was hailed by business groups and slammed by some union leaders and health experts.

Covid is here to stay. How will we know when it stops being special?

The plan to further loosen isolation guidance when the science around infectiousness has not changed is likely to prompt strong negative reaction from vulnerable groups, including people older than 65, those with weak immune systems and long-covid patients, CDC officials and experts said.

Doing so “sweeps this serious illness under the rug,” said Lara Jirmanus, a clinical instructor at Harvard Medical School and a member of the People’s CDC, a coalition of health-care workers, scientists and advocates focused on reducing the harmful effects of covid-19.

Public health officials should treat covid differently from other respiratory viruses, she said, because it’s deadlier than the flu and increases the risk of developing long-term complications . As many as 7 percent of Americans report having suffered from a slew of lingering covid symptoms, including fatigue, difficulty breathing, brain fog, joint pain and ongoing loss of taste and smell, according to the CDC.

The new isolation recommendations would not apply to hospitals and other health-care settings with more vulnerable populations, CDC officials said.

While the coronavirus continues to cause serious illness, especially among the most vulnerable people, vaccines and effective treatments such as Paxlovid are available. The latest versions of coronavirus vaccines were 54 percent effective at preventing symptomatic infection in adults, according to data released Feb. 1, the first U.S. study to assess how well the shots work against the most recent coronavirus variant. But CDC data shows only 22 percent of adults and 12 percent of children had received the updated vaccine as of Feb. 9, despite data showing the vaccines provide robust protection against serious illness .

Coronavirus levels in wastewater i ndicate that symptomatic and asymptomatic infections remain high. About 20,000 people are still hospitalized — and about 2,300 are dying — every week, CDC data show. But the numbers are falling and are much lower than when deaths peaked in January 2021 when almost 26,000 people died of covid each week and about 115,000 were hospitalized.

The lower rates of hospitalizations were among the reasons California shortened its five-day isolation recommendation last month , urging people to stay home until they are fever-free for 24 hours and their symptoms are mild and improving. Oregon made a similar move last May.

California’s state epidemiologist Erica Pan said the societal disruptions that resulted from strict isolation guidelines also helped spur the change. Workers without sick leave and those who can’t work from home if they or their children test positive and are required to isolate bore a disproportionate burden. Strict isolation requirements can act as a disincentive to test when testing should be encouraged so people at risk for serious illness can get treatment, she said.

Giving people symptom-based guidance, similar to what is already recommended for flu, is a better way to prioritize those most at risk and balance the potential for disruptive impacts on schools and workplaces, Pan said. After Oregon made its change, the state has not experienced any disproportionate increases in community transmission or severity, according to data shared last month with the national association representing state health officials.

California still recommends people with covid wear masks indoors when they are around others for 10 days after testing positive — even if they have no symptoms — or becoming sick. “You may remove your mask sooner than 10 days if you have two sequential negative tests at least one day apart,” the California guidance states.

It’s not clear whether the updated CDC guidance will continue to recommend masking for 10 days.

Health officials from other states told the CDC last week that they are already moving toward isolation guidelines that would treat the coronavirus the same as flu and RSV, with additional precautions for people at high risk, said Anne Zink, an emergency room physician and Alaska’s chief medical officer.

Many other countries, including the United Kingdom, Denmark, Finland, Norway and Australia, made changes to isolation recommendations in 2022. Of 16 countries whose policies California officials reviewed, only Germany and Ireland still recommend isolation for five days, according to a presentation the California public health department gave health officials from other states in January. The Singapore ministry of health, in updated guidance late last year, said residents could “return to normal activities” once coronavirus symptoms resolve.

Even before the Biden administration ended the public health emergency last May, much of the public had moved on from covid-19, with many people having long given up testing and masking, much less isolating when they come down with covid symptoms.

Doctors say the best way for sick people to protect their communities is to mask or avoid unnecessary trips outside the home.

“You see a lot of people with symptoms — you don’t know if they have covid or influenza or RSV — but in all three of those cases, they probably shouldn’t be at Target, coughing, and looking sick,” said Eli Perencevich, an internal medicine professor at the University of Iowa.

Journal Retracts Studies Cited in Federal Court Ruling Against Abortion Pill

The journal found that the studies, which had suggested that medication abortion is unsafe, included incorrect factual assumptions and misleading presentation of the data.

An academic journal publisher this week retracted two studies that were cited by a federal judge in Texas last year when he ruled that the abortion pill mifepristone should be taken off the market .

Most of the authors of the studies are doctors and researchers affiliated with anti-abortion groups, and their reports suggested that medication abortion causes dangerous complications, contradicting the widespread evidence that abortion pills are safe .

The lawsuit in which the studies were cited will be heard by the Supreme Court in March. The high court’s ruling could have major implications for access to medication abortion, which is now the most common method of pregnancy termination.

The publisher, Sage Journals, said it had asked two independent experts to evaluate the studies, published in 2021 and 2022 in the journal Health Services Research and Managerial Epidemiology, after a reader raised concerns.

Sage said both experts had “identified fundamental problems with the study design and methodology, unjustified or incorrect factual assumptions, material errors in the authors’ analysis of the data, and misleading presentations of the data that, in their opinions, demonstrate a lack of scientific rigor and invalidate the authors’ conclusions in whole or in part.”

The publisher also retracted a third study by many of the same authors that was published in 2019 in the same journal, which did not figure in the mifepristone lawsuit.

Sage said that when it had begun examining the 2021 study, it confirmed that most of the authors had listed affiliations with “pro-life advocacy organizations” but had “declared they had no conflicts of interest when they submitted the article for publication or in the article itself.”

Sage said it had also learned that one of the reviewers who evaluated the article for publication was affiliated with the Charlotte Lozier Institute, the research arm of Susan B. Anthony Pro-Life America.

The institute denied that the studies were flawed, as did the lead author, James Studnicki, who is vice president and director of data analytics at the institute.

“Sage is targeting us,” Dr. Studnicki, who has a doctor of science degree and a master’s degree in public health, said in a video defending the team’s work.

Noting that the studies had been used in legal actions, he said: “We have become visible, people are quoting us, and for that reason we are dangerous, and for that reason they want to cancel our work. What happened to us has little or nothing to do with real science and has everything to do with political assassination.”

In a statement, Dr. Studnicki said, “The authors will be taking appropriate legal action,” but he did not specify what that would be.

The lawsuit seeking to bar mifepristone — the first pill in the two-drug medication abortion regimen — was filed against the Food and Drug Administration by a consortium of groups and doctors who oppose abortion. In fighting the lawsuit, the federal government has defended its approval and regulation of mifepristone, provided years of evidence that the pill is safe and effective and argued that the plaintiffs have no legal standing to sue because they are not abortion providers and have not been harmed by mifepristone’s availability.

In his opinion last April, Judge Matthew J. Kacsmaryk cited the 2021 study to support his conclusion that the plaintiffs had legal standing to sue. That study reported a higher rate of emergency room visits after medication abortions than after procedural abortions. Citing it, Judge Kacsmaryk wrote that the plaintiffs “have standing because they allege adverse events from chemical abortion drugs can overwhelm the medical system and place ‘enormous pressure and stress’ on doctors during emergencies and complications.”

In another section of his ruling, Judge Kacsmaryk cited the 2022 study, writing that “plaintiffs allege ‘many intense side effects’ and ‘significant complications requiring medical attention’ resulting from Defendants’ actions.”

Judge Kacsmaryk’s opinion was criticized by many legal experts, and an appeals court struck parts of it but said significant restrictions should be placed on mifepristone that would prevent it from being mailed or prescribed by telemedicine.

Legal experts said it was unclear if Sage’s action would affect the Supreme Court’s decision. Mary Ziegler, a law professor at the University of California, Davis, said the retractions might simply “reinforce a position they were already ready to take.”

For example, she said, there were already strong arguments that the plaintiffs lacked legal standing, so if a justice was “willing to overlook all that other stuff, you may be willing to overlook the retractions too,” she said. For justices already “bothered by various other problems with standing, you probably were potentially going to say the plaintiffs didn’t have standing as it was.”

Similarly, she said, some justices would already have concluded that the vast majority of studies show mifepristone is safe, so if a justice was “prepared to say that, notwithstanding the weight of the evidence, mifepristone is really dangerous, you could easily do that again if you lose a couple of studies.”
