A Request Letter is a written document sent to the person or company that can help the sender of the letter solve various troubles and issues. Whether you need to borrow money from an acquaintance, ask your teacher for another chance to pass an exam, or arrange a meeting with someone, consider this letter to make a formal and concise request which will demonstrate your serious intentions and illustrate the problem you are facing.

If you politely express your wishes while knowing the recipient of the letter is able to help you, a Request Letter template modified to your personal needs will be successful. The documents below are commonly used to ask business partners, colleagues, teachers, and potential associates for their help, including financial contributions:

  • Donation Request Letter . Send a message to potential donors of your charity - individuals and companies alike - to ask them whether they are interested in making a one-time donation or donate to your organization multiple times, usually in exchange for a gift card or an invitation to the charitable event.
  • Payment Request Letter . You can demand payment from the person or entity that owes money to you or your business - remind them of their financial obligation before you think about legal proceedings.
  • Sponsorship Request Letter . Complete this document to gain the support of the organization - for instance, a successful business looking to improve their reputation and help with charitable causes - and ask them to become your official sponsor for a specific event or everyday activities.
  • Request for Recommendation Letter . If you need to obtain a reference from a person who has known you for a long time and is willing to vouch for your professionalism and integrity, fill out this document.

Follow these steps to compose a formal Request Letter:

  • Complete and send this document as soon as you need assistance - give the other person time to contemplate your request and find resources to help you.
  • If you send a letter via email, include the subject matter in the appropriate field.
  • Salute the recipient of the letter and introduce yourself.
  • State the purpose of your letter in the first paragraph. You can mention only the proposition - it is recommended to elaborate on the issue later.
  • Refer to your previous communication - it is possible you have discussed the matter before, and you are making a request based on that conversation, which can increase your chances for a positive answer.
  • Explain the situation at hand and emphasize the negative consequences that might occur without the help of the addressee. Do not be vague - you must leave no room for interpretation, and the request has to be straightforward enough for the individual to understand it and be prepared to assist you.
  • Attach documentation that supports your request - for example, you may ask for donations to cover bills your charity has to pay and provide copies of the bills to prove the need for assistance.
  • Be professional and polite - do not push too hard and use the appropriate tone, especially if you address a person you will depend on regardless of the request.
  • Offer to stay in contact if the recipient has further questions or needs to learn more about the problem to help.
  • Thank the other party for their time and sign the letter.

Letter to Request Documents

Rating: 4.8 - 43 votes

A Letter to Request Documents is just what it sounds like: a letter written by someone that wants to request documents (or records) from another source. Most often, this document is used to request legal documents or records from a court of law, but it does not have to be. It can be used in almost any situation where copies of certain documents or records are needed.

The one instance this letter is not adapted for is in the case of a request for medical records. In that case, a specific Medical Records Request is the best choice, which contains all the information needed to access personal medical files.

This document can be used either to request documents from a specific courthouse or from any other source . If legal records are being requested from a court of law, the form contains all the information needed to ensure the courthouse that is the recipient of the letter can easily find and access the case.

How to use this document

This document gives several options on the type of document or record that is being requested. The most common documents are judgment records from a court, transcripts from a court, a deposition, a docket record, a traffic record, or spousal documents like a marriage certificate, divorce decree, or annulment papers.

If one of the above-named documents or records are not what the sender of the letter is looking to request, the form gives the option to input the description of another document or record, with all the details the sender requires.

After this document is filled out, it is a good idea to have it printed and signed and then sent to the source of the documents or records. Many times, courthouses require payment and a self-addressed stamped envelope. Other sources may as well. If so, these details can be checked with the source itself , such as through the court website or by calling.

Applicable law

There are no specific laws that govern what must be put into a Letter to Request Documents. However, the source that the document is being requested from, such as the courthouse, may have specific rules about what needs to be included.

How to modify the template

You fill out a form. The document is created before your eyes as you respond to the questions.

At the end, you receive it in Word and PDF formats. You can modify it and reuse it.

Steps in Writing a Request Letter

As you now know, a request letter can help you get what you need, e.g. schedule an interview , borrow official equipment or materials, acquire information, etc. Thus, it is important to be able to write a request letter that will clearly and coherently convey your message. Listed below is an easy guide on how you can write an effective request letter:

1. Write out the full mailing address of the sender:

The return address of the letter should be on the upper most part of the paper, left or right side depending on the format followed. It should contain the printed name of the sender followed by the complete mailing address and contact information such as email address and/or phone number.

2. Date when the letter is written:

It is important to include the date when you have written the letter in order for you to keep track when of the days when the recipient responds. Aside from that, it helps the recipient of the letter take note of the day when the letter arrives on their end, it can help set priorities of the letters they receive, in a way.

3. Include full mailing address of the recipient:

It is important to write out the complete mailing address of the recipient or receiver of the letter to make sure the letter is given to the right person or company. In this sense, the inside address must contain the name of the receiver, mailing address, and contact information. Regardless of letter format , the inside address is commonly placed on the left side of the paper.

4. Indicate subject line:

The subject line of your letter must briefly convey the topic to be discussed further in the letter. The subject line is usually only a few words long.  As indicator, you can include the word Subject before stating the subject of the letter. The terms  Re and In re should only be used for legal correspondence.

5. Write out salutation:

The salutation of your letter must be accurate and courteous. Regardless if the letter is formal or informal, the salutation “Dear” must be used. After the salutation “Dear,” the preferred title and last name of the direct letter recipient must be written. For a formal business letter , the correct punctuation after the salutation is a colon (:).

6. Briefly state introduction:

The introduction of your letter must be able to clearly and briefly explain the purpose of the letter. You should immediately write out your intention to request something from the recipient. In the same paragraph, you should specify the date as well as the reason for your request as well.

7. Include supporting details, if necessary:

In some cases, supporting details are not really that necessary for request letters. However, in other circumstances the recipient may require it. In this case, write the supporting details on the paragraph following the introduction. The supporting details can be pertinent information regarding how a decision to give a request was made, reason/s why the request is completely necessary, what the requested material or information will be used for, etc.

8. Politely conclude your letter:

As a conclusion of your request letter, make sure you express gratitude to the recipient for the taking their time to consider your request. In the same paragraph, you can also include or reiterate your contact information should they need to discuss your request further with you.

9. Include a complimentary close and signature line:

For the same reason why there is a salutation, there should also be a complimentary close for your request letter. It should convey your professionalism and courtesy. A few spaces after the complimentary close, the signature line should come after. It should contain your printed name and title/designation.

10. Indicate enclosure and copy notations:

If you have another document attached with your request letter, you should indicate it by using the enclosure notation. And if you have given another copy of the letter to someone, say the office secretary, indicate by putting a copy notation; for example: “CC:  name of the person:”  after the enclosure notation.

  • Sincerely: This is a widely accepted and formal way to close a letter. It’s appropriate for most types of requests.
  • Yours truly: Similar to “Sincerely,” this is a formal and polite closing that can be used for various types of requests.

  • Kind regards : This closing is friendly and appropriate for formal or semi-formal requests.
  • Respectfully: This is a formal closing that is suitable for more serious or respectful requests, such as formal business or official letters.
  • Yours faithfully: This closing is often used in formal business or legal contexts, particularly in the UK.
  • Thank you: If you want to convey your gratitude explicitly, you can simply end with “Thank you” followed by a comma.

After your chosen closing phrase, leave a space and then type or write your name. If it’s a physical letter, you can sign your name by hand above your typed name. If it’s an email, you can simply type your name below the closing phrase.

General Tips for Writing a Letter of Request

You can increase the chances of the recipient of your letter agreeing to what you ask them to do by following a few simple steps for writing business letters . The following suggestions will help you write an effective letter of request:

  • Use an appropriate business letter format.
  • Keep it simple. In the first paragraph, tell the recipient why you are writing.
  • If appropriate, provide the recipient with pertinent information to help them remember who you are. For example, if you are writing to a former professor, explain what class you were in and the year. In the case of writing to a former supervisor, remind the person when you worked with them. These details help readers place where they know you from.
  • Briefly explain what it is you want the reader to do. If there is a deadline involved, share that information as well.
  • Give the reader all the information they need to comply with your request.
  • Include any supporting documentation the recipient may need.
  • List your contact information, including full name, mailing address, phone number and e-mail address in the body of the letter. Ask the reader to contact you if they have any questions or concerns about your request.
  • Thank the person for his or her consideration.
  • Bring the letter to its conclusion by using an appropriate closing for professional correspondence.

Consider the Reader

When you write your letter of request, put yourself in the shoes of the person who will read it. This is important whether you're sending a letter of request to a supplier, customer, employee, or other individual. Proofread a draft of your letter to ensure it is free of errors and makes sense as written. Double check that the reader won't need any further information to decide whether they can say yes to your request. Make any changes necessary before sending the letter.

Create a form in Word that users can complete or print

In Word, you can create a form that others can fill out and save or print.  To do this, you will start with baseline content in a document, potentially via a form template.  Then you can add content controls for elements such as check boxes, text boxes, date pickers, and drop-down lists. Optionally, these content controls can be linked to database information.  Following are the recommended action steps in sequence.

Show the Developer tab

In Word, be sure you have the Developer tab displayed in the ribbon.  (See how here:  Show the developer tab .)

Open a template or a blank document on which to base the form

You can start with a template or just start from scratch with a blank document.

Start with a form template

Go to File > New .

In the  Search for online templates  field, type  Forms or the kind of form you want. Then press Enter .

In the displayed results, right-click any item, then select  Create.

Start with a blank document

Select Blank document .

Add content to the form

Go to the  Developer  tab Controls section where you can choose controls to add to your document or form. Hover over any icon therein to see what control type it represents. The various control types are described below. You can set properties on a control once it has been inserted.

To delete a content control, right-click it, then select Remove content control  in the pop-up menu.

Note:  You can print a form that was created via content controls. However, the boxes around the content controls will not print.

Insert a text control

The rich text content control enables users to format text (e.g., bold, italic) and type multiple paragraphs. To limit these capabilities, use the plain text content control .

Click or tap where you want to insert the control.

Rich text control button

To learn about setting specific properties on these controls, see Set or change properties for content controls .

Insert a picture control

A picture control is most often used for templates, but you can also add a picture control to a form.

Picture control button

Insert a building block control

Use a building block control  when you want users to choose a specific block of text. These are helpful when you need to add different boilerplate text depending on the document's specific purpose. You can create rich text content controls for each version of the boilerplate text, and then use a building block control as the container for the rich text content controls.

building block gallery control

Select Developer and content controls for the building block.

Insert a combo box or a drop-down list

In a combo box, users can select from a list of choices that you provide or they can type in their own information. In a drop-down list, users can only select from the list of choices.

combo box button

Select the content control, and then select Properties .

To create a list of choices, select Add under Drop-Down List Properties .

Type a choice in Display Name , such as Yes , No , or Maybe .

Repeat this step until all of the choices are in the drop-down list.

Fill in any other properties that you want.

Note:  If you select the Contents cannot be edited check box, users won’t be able to click a choice.

Insert a date picker

Click or tap where you want to insert the date picker control.

Legacy control button

Select the Legacy Form control or Active X Control that you want to include.

Set or change properties for content controls

Each content control has properties that you can set or change. For example, the Date Picker control offers options for the format you want to use to display the date.

Select the content control that you want to change.

Go to Developer > Properties .

Controls Properties button

Change the properties that you want.

Add protection to a form

If you want to limit how much others can edit or format a form, use the Restrict Editing command:

Open the form that you want to lock or protect.

Select Developer > Restrict Editing .

Restrict editing button

After selecting restrictions, select Yes, Start Enforcing Protection .

What Is a Request Letter?

A request letter is a letter that is written with the sole purpose of asking another individual, organization, government agency, or any business entity to do something or approve of something. There is a necessity for the drafting of request letters since this is normally considered as a formal and proper method of making a request in the professional setting. Far be it for individuals to make requests verbally or in an informal way.

What Are the Benefits of Writing Request Letters?

As mentioned, requests that are being done in the professional setting must be done formally through a proper request letter. The items below demonstrate some sound arguments as to why it is beneficial to send in a request letter:

  • A request letter is just like any business letter or formal letter . It follows the standard business letter format. This makes it easier for the recipient to treat the content of the request in a more professional manner as opposed to receiving a verbal request, which can easily slip from their minds in a matter of minutes.
  • A request letter allows one to go straight to the point of their request without sounding too assertive. This is because request letters are simply designed to illuminate the recipient’s minds as what the request letter is all about. There is no room for excessive formalities and niceties. Starting one’s letter with a simple greeting is enough to satisfy the social niceties that are inherently required in any type of social interaction or communication. This is extremely more convenient as compared to a verbal communication of a request in which one has to exchange a few sentences of formal introductions before going to the heart of the matter.
  • A request letter is informative. It would be easier for the recipient to assess whether or not he/she should grant the request based on the information provided in the letter. A request letter not only states the intention of the writer to ask for something but also allows him/her to extrapolate on the argumentative points explaining why his/her request must be granted.
  • A request letter is bereft of any grammatical errors as formal letters are proofread. There is a sufficient amount of time that is allotted for the writing of any business letter. This makes the room for error and the inclusion of unnecessary information smaller as compared to hastily done verbal requests.

What Are the Essential Parts of a Request Letter?

Below are the items that must be included in any request letter:

  • The return address
  • The address and complete name of the recipient
  • The salutation
  • The brief and formal greeting
  • The statement of the purpose of the request
  • The explanation of why a request must be granted
  • The sentences seeking or hoping for an immediate and positive response from the recipient
  • The mentioning of supporting documents that are attached to the request letter
  • The closing remark
  • The name and signature of the letter-writer.

When Can You Use a Request Letter?

A request letter format is so general that it can be written for numerous purposes. Below is a description of circumstances when a request letter must be written:

  • It is written by a landowner or an owner of any property that is being leased in order to formally request a tenant to make his/her rental payment.
  • It is written by creditors to formally ask their debtors to pay their debts. This includes personal creditors (or those who are closely related to their debtors), banks, and other lending institutions.
  • It is written by businesses to other businesses or business owners in order to formally make an initial sample proposal to discuss the possibilities of forming a business partnership.
  • It is written by nonprofit organizations to charitable entities for the purpose of asking them for a donation.
  • It is written by businesses to their customers who have failed to make their payment on time. This is a formal and less assertive way of requesting for payment, which is suitable as an initial step before a business can proceed with writing an actual demand letter that lays down the legal consequences should the customer still fail to pay. To know more about demand letters , check out our website’s wide collection of informative demand letter templates.
  • It is written by the administrators of a university who are reviewing the qualifications of a student-applicant who wants to study at their prestigious educational institution. For the purpose of examining the qualifications of an applicant, school administrators send a request letter to the previous professors of the applicant who can provide reliable sample references regarding the academic standing and other valuable traits of the applicant.
  • It is written for the purpose of canceling the continuation of services.
  • It is written for the purpose of setting up an appointment with an individual, a professional, or a business owner.
  • It is written by human resource managers to the previous employers of a job applicant. This is called a reference request letter .
  • It is written by individuals, organizations, and businesses to formally request a bank or any financial institution to grant them a loan.
  • It is written by customers who are dissatisfied with a service or have received a defective product or good for the purpose of asking for a refund from the business.
  • It is written by customers who are requesting that the goods they have paid for would be delivered in case it has not been delivered on the date agreed.
